The form is filled out on one sheet from both sides. On the first page you must specify your surname, name and patronymic in the nominative case. Next, write the date of birth: the number of digits, and the name of the month – in words.
In the following paragraph, write the name of the country whose citizen you are. To indicate the place of birth on the form have multiple lines. You need to fill the ones that suit you. Write the name of the Republic/Krai/oblast or district, opposite the corresponding row. Then specify the district and the city or village.
In the seventh paragraph, underline marking the floor, and in the eighth, on the contrary, cross out the unwanted option (registration at the place of stay or place of residence).
Then enter directly the address where you are registered and the full name of the authority which issued the check. In addition, you need to specify your passport data: series and number of passport, issuing authority and date of receipt.
On the second page of items to fill much less. Here you are asked to specify the location from which you arrived at a new address. If you simply moved from one residence to another within the same locality, write in the appropriate column the old address. In that case, if you have changed personal data – surname, name, patronymic old you also need to write.
At the end, specify the date sheet arrival, the name of the month at the same place in words. In the same paragraph there is a space provided for your signature.
Many online sites place forms sheets of arrival, available for free or paid download. However, they often do not meet the requirements for the design of the document (often distorted the size of the form and placement of lines with questions). Therefore, it is always safer to use the forms that you will be given the authority of the Federal migration service.
